Patient Forms

If you are a new patient, we'll spend time with you at your first appointment discussing your health history, health privacy, and dental insurance benefits.  Printing and filling out the forms before your arrival gives you more time to provide accurate information at your leisure. Please arrive early to your appointment if you plan to fill out the forms in our office. If you have daily medications, please bring the list with you to your first appointment. We're here to help answer any questions at 913-227-0466.
